Health 2.0 - Post-Discharge Care Coordination Platform for Patients with Chronic Conditions is a care coordination platform that helps patients determine the efficacy of their treatment and help them stay the course. In order to promote the benefits of active management of health and the idea of prevention being better than the cure, we would like to announce the formation of the "Health & Technology 2.0 Scholarship".

This scholarship will support students in Canada and the United States to achieve their goals academically who have first hand experience regarding the topics discussed below or know anyone that has benefitted from using technology as a means to better manage their health. This scholarship hopes to support one (1) student a year with the cost of tuition by awarding them a $1200 scholarship to the successful applicant.

Interested participants must meet the following criteria:

Currently enrolled in the Fall / Spring school year of 2016/2017 at any college, university, high school, trade or technical school within the United States of America or Canada.
Students must have a cumulative Grade Point Average (GPA) of at least 3.0 (or equivalent)
Students must be 15 years or older at the time of enrollment
Must be an American or Canadian citizen or hold a valid student visa
If you are interested in participating, please choose ONE (1) of the following prompts to answer in the essay submission:

  1. Describe the importance of post-discharge care coordination for improving and maintaining good health
  2. Describe why tracking one's health using an electronic platform may lead to faster recovery time and less cost.
  3. Explain your thoughts on technology improving patients quality of life especially those dealing with chronic conditions.
  4. Explain how tracking and quantifying your self can improve your health in the long run.
  5. Explain and discuss with evidence how someone you know has directly benefited from Herbs, Supplements to manage any health issues successfully.

In your essay, please include the following:

A title
Your article in Google Docs with a minimum of 500 words and maximum of 850
